Bay Zoltán Applied Research Nonprofit Ltd. participates in the Personalized ICT solution to reduce re-hospitalization rates in heart failure elderly patients suffering from comorbidies (PerHeart) .

 

The PerHeart project aims to develop an infocommunication technology that can help reduce the rate of repeated hospitalisations for patients with heart disease.

To this end, it supports patients in adhering to therapy and lifestyle changes by providing personalised feedback, identifying risk factors, and analysing patient profiles and providing data to help professionals interpret and predict disease.

The PerHeart system will be tested and validated over 14 months on the basis of data collected in pilot studies (pilot operation) in three countries (Poland, Denmark, Italy), involving 50 patients with heart disease discharged from hospital and their carers in each country.

Duration of the project: 2020.09.01. – 2023.12.31.
